The Notice of Related Cases form is required to let the county know if you currently have any open cases in the court system. Even if you do not have an open case, please fill out the form as many counties will still require it.
A related case is defined as "an open or closed civil, criminal, guardianship, domestic violence, juvenile delinquency, juvenile dependency, or domestic relations case".
In other words, the courts would like to know if you have other cases in the court system so that they can consider those cases as well when deciding whether to approve your name change request.
