step 1 - petition for name change

Please note – Step 1 (Petition for Name Change) is necessary for changing your legal name with the state of Florida. It does not need to be completed if you are keeping your current name (or you have already changed your legal name) and are only looking to update your gender marker. 
 
If you are only looking to update your gender marker, you need to skip to step 2 of the process below (No - Update Social Security Card).  A doctor's letter is required to update the gender marker on your social security card, driver license, passport, or birth certificate.  The processes to update you gender marker on these documents will be fully explained in steps 2 to 5.
