Fill out Notice of Related Cases

The Notice of Related Cases form is required to let the county know if you currently have any open cases in the court system.  Even if you do not have an open case, please fill out the form as many counties will still require it.  

A related case is defined as "an open or closed civil, criminal, guardianship, domestic violence, juvenile delinquency, juvenile dependency, or domestic relations case".​​

In other words, the courts would like to know if you have other cases in the court system so that they can consider those cases as well when deciding whether to approve your name change request.

We'll ask you one question at a time and your answers will fill out the Notice of Related Cases form​.  Then the form will be emailed to you.
Information Needed​

  • Information regarding any open or closed court cases that involve the same petitioners, respondents, children, or issues and is pending at the time the name change case is being filed
    • For example; the same petitioner and respondent could have a custody case in progress that involves the same minor that is petitioning to change their name.  This would be a related case as the same parties and children are involved in both.
  • You will need to provide the following information for any related cases:
    • The name(s) that are on the case
    • Petitioner(s) of the case (who filed it)​
    • Respondent of the case (if any)
    • The case number and division
    • The type of case (i.e. Custody, Guardianship, etc...)
    • The state and county that the case is filed in
    • How the other case relates to the new name change case
