A letter from a physician is needed for someone to update the gender marker on their Social Security, Driver License, Passport, and Birth Certificate documentation.  This letter is referred to as a "Medical certification of appropriate clinical treatment" and verifies that the patient has completed or is in the process of clinically transitioning from one gender to another. 

The automated form below allows physicians to use our system to generate these letters for their patients.  Our form will walk you through a short-guided interview process and will then email the letter packet to you in PDF format.  You can then print these letters on your practice's letterhead (we can provide this as part of the document if you don't already have your own), sign them, and give them to your patients so they can complete the needed updates for their documentation.    
Please note - The Physician's Letter form will generate a PDF packet with four versions of the letter in it.  There is a specific letter format for each type of documentation your patient may be updating (Social Security, Driver License, Passport, and Birth Certificate).  It is important that you sign all four forms, and that your patient uses each letter to only update the document that is listed on the bottom right corner.    

​If your patient tries to use one of the letters to update a document that it was not specifically written for, it is possible that their update request will be rejected by the administering agency.
