Fill out WAIVER OF PERSONAL APPEARANCE FORM

Santa Rosa county allows you the option of not having to appear before the judge and court for your name change case.  This form is not required if you wish to appear before the court in person.

Download and print the form below, fill it in with black ink, and set it aside.  You will need this form when it is time to file all the paperwork.

Form Tips
  • For "Petitioner" enter your current name.
  • For "CASE No." leave blank.
  • For "Division" leave blank.
  • Please fill out your current full legal name and the new name you are changing to in the body of the form (line 1 and immediately above).
  • Please sign on the "Signature of Petitioner" line, and put the current date in the "Dated" field.
  • Please be aware that this form needs to be notarized before it is returned to the Santa Rosa Clerk of Court's office.
