step 1 - petition for name change

It is very important to do this step first!
In this step, you will file a petition with the court to change your legal name and receive a signed order from a judge.

Then in Steps 2 through 5 you will use this signed order to change your name on your official government identification.

Please note - There are certain forms that everyone seeking a legal name change must fill out, regardless of which county you live in. You will first fill out those forms, and then you will be prompted to select the county in which you reside, at which point you will fill out any specific forms required by your county. Don't worry, just follow the steps!
Is the person changing their name an adult or minor?
